Severe Maternal Morbidity (SMM) Review Form, Council on Patient Safety in Women’s Health Care
The Council on Patient Safety in Women’s Health Care provides a Severe Maternal Morbidity reporting form, in both a long and a short version. To access these forms, you must first create a free user account. The Severe Maternal Morbidity Review Form is found under the toolbar “Patient Safety Bundles and Tools,” and then by selecting “Patient Safety Tools,” from the dropdown.
