CDC issues updated H1N1 intrapartum and postpartum treatment guidance for hospitals.

This interim guidance has been updated to replace previously posted guidance entitled "Considerations Regarding Novel H1N1 Flu Virus in Obstetric Settings", dated July 6, 2009. This document clarifies clinical considerations related to management of suspected or confirmed maternal infection with 2009 H1N1 influenza virus infection within labor and delivery, postpartum, and newborn care settings in hospitals. A cautious approach to the management of ill mothers and their newborns is still recommended, but several options are provided based on hospital configuration, staffing, and surge capacity.

The November issue of Monthly Member Resources and Information is now online.

One of the most effective ways to "go green" is to consider the potential environmental impact of each printed piece, taking into consideration paper weight, mail format, the emissions of the trucks delivering the materials, and the need for envelopes. ACOG continues to think about ways to practice being environmentally friendly and therefore, has made the decision to go on-line with the ACOG Member Resource packages. We know that reducing the impact of mailing 55,000 envelopes each month, which inevitably end up in the recycle bin or a landfill, will make a difference. Members will continue to receive the same materials to which they have become accustomed, however, they will now available electronically.

The Mayo Clinic is recruiting participants for a randomized clinical trial to compare the safety and effectiveness of two standard fibroid treatments: MRI guided ultrasound surgery (MRgFUS) and uterine artery embolization (UAE).

The primary goal of this study is to compare the safety and effectiveness of two standard fibroid treatments: MRI guided ultrasound surgery (MRgFUS) and uterine artery embolization (UAE). Both treatments are approved by the Food and Drug Administration (FDA) for women who do not plan to become pregnant. A second goal of this study is to better understand which symptoms bother women with fibroids the most. Understanding and addressing the symptoms of clinically-significant uterine fibroids is important in order to optimize treatment outcomes and control health care costs. Women who are eligible to participate in the study, are randomized to one of two treatment arms (UAE or MRgFUS). Women in both arms will receive treatment but will not be able to choose which treatment she will receive. A comprehensive assessment of symptoms-including experienced pain- will take place at baseline, 6 weeks, and 6 months following treatment with UAE and MRgFUS.

On October 20th College President Gerald F. Joseph, Jr., MD FACOG issued another important update about H1N1 ("swine flu") to Fellows.

More pregnant women are dying from H1N1 (formerly "swine flu") as compared to the general population. The best strategy to prevent this is to vaccinate pregnant women for both 2009 H1N1 influenza and seasonal flu. I strongly encourage you to vaccine your pregnant patients. But most importantly, immediately treat pregnant patients who have influenza-like illness. For pregnant patients with influenza-like illness, do not wait for test results to initiate treatment, if testing is performed.

Pregnant Women and the Flu.

This 2009-2010 influenza season, there are two different types of flu to avoid -- 2009 H1N1 ("swine flu") and seasonal flu. If you are pregnant, the flu can be very serious for both you and your baby. Some pregnant women sick with H1N1 flu have had early labor and severe pneumonia. Some have been hospitalized and some have died.

President Dr. Gerald F. Joseph, Jr issues important information about H1N1 and pregnant women.

Healthy pregnant women have been severely impacted by the 2009 H1N1 influenza virus (formerly called "novel H1N1 flu" or "swine flu"). Pregnant women are 4 times as likely as the general population to be hospitalized for the 2009 H1N1 influenza virus. Six percent of confirmed deaths due to H1N1 flu have been in pregnant women while only 1% of the general population is pregnant. Obstetrician-gynecologists can play a pivotal role in helping to prevent these adverse outcomes.

On July 6, CDC posted updated guidelines titled, "Considerations Regarding Novel H1N1 Flu Virus in Obstetric Settings".

This document has been developed to provide guidance for prevention and management of novel H1N1 flu infection in inpatient and out-patient obstetric settings. Severe illnesses among pregnant woman and infants have been reported in this outbreak, although the epidemiology and spectrum of illness among pregnant woman and infants are not fully understood at this time and are under investigation. Prevention of infection with novel influenza A (H1N1) virus in pregnant women and infants is the highest priority message in this guidance. Healthy pregnant women should have access to prenatal care and labor and delivery services in settings where they can be separated from persons who are ill or potentially ill with novel H1N1 flu infection. In addition, a cautious approach to the management of neonates with ill mothers is recommended which includes isolation and close observation. Recommendations are interim, based on current knowledge of the novel H1N1 flu outbreak in the United States, and may be revised as more information becomes available.

On June 30, 2009 the Centers for Disease Control and Prevention released new guidelines for clinicians on treatment of pregnant women infected with novel influenza A (H1N1) (formerly known as swine flu).

This document provides updated, interim guidance on the use of influenza antiviral treatment of pregnant women who are sick with novel influenza A (H1N1). The highest priority message is to treat pregnant women with influenza-like illness as soon as possible; treatment should not be withheld pending results of testing for influenza, if testing is done. Influenza antiviral chemoprophylaxis recommendations have been updated to be consistent with CDC recommendations for chemoprophylaxis for high risk groups. Finally, infant feeding recommendations have been updated to reflect current mask use guidance and the need for a cautious approach to preventing infection in infants, even though clinical data are lacking. Recommendations are interim, based on current knowledge of the H1N1 outbreak in the United States, and may be revised as more information becomes available.

CDC has published case reports on pregnant women with H1N1.

CDC first identified cases of respiratory infection with a novel influenza A (H1N1) virus in the United States on April 15 and 17, 2009 (1). During seasonal influenza epidemics and previous pandemics, pregnant women have been at increased risk for complications related to influenza infection (2-5). In addition, maternal influenza virus infection and accompanying
hyperthermia place fetuses at risk for complications such as birth defects and preterm birth (6). As part of surveillance for infection with the novel influenza A (H1N1) virus, CDC initiated surveillance for pregnant women who were infected with the novel virus. As of May 10, a total of 20 cases of novel influenza A (H1N1) virus infection had been reported among pregnant women in the United States, including 15 confirmed cases and five probable cases.* Among the 13 women from seven states for whom data are available, the median age was 26 years (range: 15-39 years); three women were hospitalized, one of whom died. This report provides preliminary details of three cases of novel influenza A (H1N1) virus infection in pregnant women. Pregnant women with confirmed, probable,
or suspected novel influenza A (H1N1) virus infection should receive antiviral treatment for 5 days. Oseltamivir is the preferred treatment for pregnant women, and the drug regimen should be initiated within 48 hours of symptom onset, if possible. Pregnant women who are in close contact with a person with confirmed, probable, or suspected novel influenza A (H1N1) infection should receive a 10-day course of chemoprophylaxis with zanamivir or oseltamivir.

NIH sponsors study of women carrying a baby with spina bifida.

MOMS is a research study designed to compare two approaches to the treatment of babies with spina bifida: surgery before birth (prenatal or fetal surgery) and surgery after birth (postnatal surgery). Spina bifida is a complex birth defect in which a portion of the spinal cord and associated nerves as well as the surrounding spinal bones and overlying muscle and skin do not fully develop. At birth the incompletely developed portion of the spinal cord protrudes through the open bones and skin. The incomplete development of the spinal cord can occur anywhere along its length, from the neck to the lower back and results in a variety of medical problems.

Learn more about FDA quarterly reports that list certain drugs under evaluation for potential safety issues.

The FDA has announced that it will post quarterly reports on its Web site that list certain drugs that are being evaluated for potential safety issues. The FDA said it will determine which drugs will be included in the reports by reviewing its adverse event reporting system (AERS). The agency said that the appearance of a drug on this list does not mean that the FDA has concluded that the drug has the listed risk or that the FDA has identified a causal relationship between the drug and the listed risk.

English and Spanish versions of Episiotomy: Procedure and Repair Techniques are now available to members online.

Episiotomy is the most common operative procedure that most obstetricians will perform in their lifetime. Because it is so common and considered minor surgery, teaching students or interns the principles and techniques usually is left to the most junior of residents. As a result, the Residency Review Committee for Obstetrics and Gynecology (RRC) asked the American College of Obstetricians and Gynecologists (ACOG) to prepare a teaching aid for all residents, but especially those with the least experience. The result is this monograph.

Learn more about the new Ralph W. Hale, MD, History Museum, what it means to the specialty, and what you can do to ensure its success.

Dr. Ralph W. Hale, ACOG's executive vice president, has had a keen interest in all things historical for a long while. For several years he wanted to find a space at ACOG for a collection of artifacts related to Obstetrics and Gynecology, some of which we already had, but were unable to adequately display. When it was decided to create a copy center last year, and the equipment was moved from its location between the Resource Center and the Fitness Room, a space opened up. Dr. Hale mentioned his plans for a history museum to Dr. Noller at one of his meetings with the presidential officers. At the Executive Board meeting held July 2007, Dr. Noller recommended that the history museum be named after Dr. Hale. The recommendation passed unanimously. He then spoke with Margaret Goodman to find out how soon the museum could be done because he really wanted to be the one to participate in the dedication of the museum at the Executive Board meeting in February 2008.
