It has been noted that some Medicare(CMS) contractors are declining payment for the routine pelvic and breast examinations associated with HCPCS code G0101 (cervical or vaginal cancer screening). Specifically, payment has been denied when the documentation includes notation of surgically absent organs as part of the seven of eleven required exam components.

The Education Committee of the Council for Resident Education in Obstetrics and Gynecology (CREOG) has developed a new self-assessment tool for residents' the CREOG Clinical Quiz. Each case contains an image and set of multiple-choice questions. The answers will be provided along with a discussion and references. Initially, cases were posted for a period of time, removed and replaced by new cases. However, due to the tremendous interest in the quiz items, CREOG has decided to continue to have all items available. In addition, it was decided to catelog the quiz items by educational objectives.

I wish to bring to your attention the CREOG resident quiz series, which is posted on the CREOG web site. This series is composed of separate learning exercises. Each exercise begins with a visual image or clinical scenario. The reader then is presented with 3-5 multiple-choice questions, followed by a list of the correct answers and a short summary paragraph and supporting reference. Each quiz can be completed within 10 minutes. I believe that you will find this quiz series to be of immediate practical value in learning basic concepts in obstetrics and gynecology.

Medicare has stated that the review of systems (ROS) and past, family and social history (PFSH) that are documented during an evaluation and management patient encounter may be collected on a history intake form that is filled out by the patient or by nursing staff. We have developed such an intake form that will meet Medicare's documentation requirements. The form includes all the information that would be necessary for a complete review of systems and a complete past, family and social history.

The International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) is currently being used in most countries. The United States is among a very few industrialized nations still using the ICD-9 code set for diagnosis code reporting. The Department of Health and Human Services (HHS) recently announced their intent to delay the International Classification of Diseases (ICD-10) compliance date. On April 17, 2012, HHS published a proposed rule that provides for a reschedule of the delayed implementation of ICD-10.

On April 8, 2013, The Joint Commission released Sentinel Event Alert Issue 50 on "Medical device alarm safety in hospitals." In the Alert, The Joint Commission notes that "while these alarm-equipped devices are essential to providing safe care to patients in many health care settings . . . [they] present a multitude of challenges and opportunities for health care organizations when their alarms create similar sounds, when their default settings are not changed, and when there is a failure to respond to their alarm signals."

On January 17, 2013, the federal Department of Health and Human Services (HHS), Office for Civil Rights (OCR), issued the long-anticipated final omnibus amendments (the "2013 Amendments") to the Privacy, Security, Breach Notification, and Enforcement Rules (the "HIPAA Rules") under the Health Insurance Portability and Accountability Act (HIPAA). The 2013 Amendments are effective as of March 26, 2013, and compliance with applicable requirements generally must be made within 180 days―by September 23, 2013 (with important exceptions for existing business associate arrangements). Significant penalties apply for non-compliance.

He or she works closely with the executive vice president and the chief financial officer in this role. The treasurer serves a three-year term on both Executive Boards and is not eligible for re-election. The treasurer is a member of both Executive Committees, the Committee on Finance and the Compensation Committee. The treasurer is responsible for reviewing selected documents for consistency with Congress or College policies and reviewing selected documents from other organizations submitted for action. ACOG’s Coding Department has assembled the latest Medicare updates for 2013 of codes normally used by ob-gyns. The charts include information concerning Medicare's 2013 Relative Value Units (RVUs) and payment policies for codes commonly reported by ob/gyns. For each procedure, information is provided on the RVUs assigned; global surgical days; whether Medicare will pay for an assistant at surgery or a co-surgeon; whether there is a payment adjustment for bilateral or multiple procedures; and payment when a physician performs only part of the global surgical package (that is, only the pre-operative or post-operative services).

The ICD-9-CM (I-9) to ICD-10-CM (I-10), and I-10 to I-9 crosswalk tools are based on the Centers for Medicare and Medicaid (CMS) General Equivalency Mapping (GEMS) documents. Per the CMS Gems guide: “The I-10 and I-9 GEMs are used to facilitate linking between the diagnosis codes in I-9 volume 1 and the new I-10 code set. The GEMs are the raw material from which providers, health information vendors and payers can derive specific applied mappings to meet their needs.”

Results are in from the 2012 ACOG Survey on Professional Liability, covering the period 2009 through 2011. This latest survey reflects an ongoing negative liability environment in that 51% of ob-gyn survey respondents have made one or more changes to their practice as a result of the affordability and/or availability of professional liability insurance, and almost 58% have made one or more changes to their practice as a result of the risk or fear of professional liability claims or litigation.

The Committee on Health Care for Underserved Women developed a Committee Opinion on Reproductive and Sexual Coercion. Reproductive and sexual coercion involves behavior intended to maintain power and control in a relationship related to reproductive health by someone who is, was, or wishes to be involved in an intimate or dating relationship with an adult or adolescent. This behavior includes explicit attempts to impregnate a partner against her will, control outcomes of a pregnancy, coerce a partner to have unprotected sex, and interfere with contraceptive methods.
