Webcast: Documentation as a Communication Tool

COURSE DESCRIPTION:
As the primary communication tool between physicians and other health care providers, the medical record is key to good patient care and is paramount to continuity of care. It is also a legal document that must withstand scrutiny. Unfortunately, physician notes may not clearly document the facts of a particular situation and demonstrate thorough work. Inadequate notes may be open to misinterpretation or may not be helpful either in providing care to the patient or in demonstrating what took place. This webcast will focus on ways to improve documentation in the medical record to communicate the patient’s status to all members of the health care team.
OBJECTIVES:
At the conclusion of this educational activity, participants will be able to:
- Explain the purposes of the medical record, including as a communication tool, for billing, to address medico-legal issues, and for retaining medical information.
- Identify how, what, and when to document, along with the basic elements of proper documentation.
- Discuss the unique issues pertaining to the use of electronic medical record.

Fee is charged per computer. There is no limit as to the number of listeners at each site. If registering several different sites, each one must register separately.
