Key Information about the Coordinated Care Initiative
Starting no sooner than April 2014, the Coordinated Care Initiative (CCI) will promote increased coordination in the health care provided to seniors and people with disabilities receiving both Medi-Cal and Medicare. The Initiative aims to help people stay at home and out of institutional settings as much as possible by creating a more person-centered way of delivering care that supports families and providers.
Beneficiaries will be able choose a Cal MediConnect plan or a plan for their Managed Long Term Services and Supports (MLTSS). This will take place in eight counties: Alameda, Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo, and Santa Clara.
It’s important to know that notices for this program will go out no sooner than January 2014. Any Department of Health Care Services mailings or notices going out to beneficiaries during fall 2013 are unrelated to this program. Beneficiaries and their families should keep this in mind when evaluating health care coverage options through the end of 2013.
