This site contains the latest news and resources about patient safety. The Classics section lists the most frequently cited information. FDA Medical Product Safety Network (MEDSUN)
MedSun provides medical device and patient safety information. There is a link to the MedSun Newsletter and device problem reports.
FDA Patient Safety News

Institute for Healthcare Improvement : Patient Safety
Founded in the late 1980s by a group of individuals committed to redesigning healthcare into a system no longer plagued by errors, waste, delays, and unsustainable social and economic costs. Its current focus is on sharing patient safety-improvement knowledge and methods. Includes discussion forums, patient safety tools, case studies/lessons learned, audio and video programs.

Managing Obstetrical Risk Efficiently (MOREOB)
This is a patient safety, professional development, and performance improvement program for hospital obstetrics units. The website has information on the MOREOB and Advancing with MOREOB programs.
National Academy for State Health Policy : Patient Safety Toolbox
This electronic toolbox is intended to provide states with tools they can use or modify as they develop or improve adverse event reporting systems. It includes information (policies, practices, forms, reports, methods, and contracts) related to states’ reporting systems, Web links, and fast facts and issues related to patient safety.
National Center for Patient Safety
Established in 1999 by the Department of Veterans Affairs for patient safety managers and officers at the VA hospitals and regional headquarters that participate in the program. The website includes publications, training iniatives, newsroom, FAQs, tools, and web resources.
National Coordinating Council for Medication Error Reporting and Prevention
NCC MERP has council recommendations on how to reduce errors, as well as phone numbers and web links to report a medical error.
National Patient Safety Foundation

Oregon Patient Safety Commission
Patient Safety Commission was created by the Oregon Legislature in July 2003. It is a "semi-independent state agency." Includes news of interest to state citizens and a petient safety reporting program.

Persons United Limiting Substandards and Errors in Health Care
PULSE is a nonprofit organization working to improve patient safety and reduce the rate of medical errors using real life stories and experiences.
