Candidate Agreement

The Candidate Agreement is to be completed by all Fellows who decide to become a candidate for national office.  The Candidate Agreement should be downloaded, signed and returned with the other materials mentioned in the national nomination procedures letter from Dr. Hal C. Lawrence III.  The documents mentioned in the agreement are available for your review on ACOG’s website.
